We the Community HealthCorps members at Salud Family Health Centers have nominated Rachel Wolf for the 2013 Service Impact Award for her creation of the Transitions of Care Program. Ms. Wolf helped create this program as a former HealthCorps member at Salud Family Health Center during her 2010-2011 service year and since then the program has become a central component to the Community HealthCorps program at Salud. Since the founding of the Transitions of Care Program, more than two thousand patients have received post discharge care coordination due to the structure created by Ms. Wolf. It is because of the tremendous impact that the TOC Program has had on the Salud patient population as well as its importance to Salud, as a federally qualified health center, that we have nominated Ms. Wolf for this year's Service Impact Award.
